Checklist to ensure your booking is complete:

	Contact Us - to make your reservation
	Pay your deposit, (Secure Credit Card Form) and you will receive a confirmation of your booking from Reservations
	Submit your Diver Profile Form - This provides us with your diving details, emergency contacts, special requirements and dive gear requirements. It is essential that this is completed prior to the expedition departure date. You may either lodge the form electronically or by printing and faxing the details to our office.
	Submit your Liability Release form - Please read the Liability release carefully as you must indicate you have read and understood the Disclaimer and are willing to sign this document before you depart on your expedition. An Email "Intention to sign" is supplied
	Pay the balance owing for your expedition, 60 days prior to travel.
	Payment and cancellation policy : A 20% deposit is required to make a booking with the balance payable 60 days prior to departure. Full payment is required for bookings less than 60 days prior to departures
	We recommend you take out travel insurance, and Emergency Medical Evacuation Assistance (see DAN Information sheet) prior to your departure.
